To schedule a complimentary consultation, you do not need a referral from your dentist. Simply call our office at (801)375-7088 and Tera, our Scheduling Coordinator, will be happy to set up a convenient appointment for you.

Tera will then send a new patient form that we would like you to complete before coming to our office. If you need driving directions, we will be happy to provide them.
